Instructions on How to Fill Out Connecticut Ed 192

Once you've determined that you meet the criteria for an Academic Skills Assessment waiver in Connecticut, you'll need to fill out the ED 192 form accurately and submit it alongside your official test scores. It's crucial that these steps are followed meticulously to ensure your waiver application is processed without delays. The Connecticut State Department of Education requires these forms to evaluate your eligibility for a waiver, allowing you to bypass the Praxis I tests for educator certification. Now, let's walk through the necessary steps to complete your application correctly.

  1. Collect your official score report: Before starting the ED 192 form, make sure you have your official SAT, ACT, GRE, or PAA score report. Remember, scores must be sent directly from the testing agency to the Connecticut State Department of Education. Personal copies or scores sent by applicants are not accepted.
  2. Fill out personal information: Use blue ink and uppercase letters to fill in your last name, first name, middle initial, social security number, complete address, gender, birth date, phone numbers, and email address. If you have a former last name, ensure it's included as well.
  3. Specify the waiver basis: Clearly mark which test scores (SAT, ACT, GRE, PAA) you are submitting as the basis for your waiver request. Provide the name of the Bachelor’s institution you are attending or have attended.
  4. Check the essential skills testing requirement: Indicate which test's score you are using to waive the academic skills assessment by checking the appropriate box. Remember to refer to the provided testing agency contact list for directions on how to have your official score report sent if you haven't done so already.
  5. Complete race/ethnicity section (optional): Although this part is optional, filling it out can help provide a fuller picture of applicant demographics.
  6. Sign and date the form: After reviewing the information for accuracy, sign and date the bottom of the form to certify that all provided information is correct to the best of your knowledge.
  7. Mail the completed form and additional required documents: Send your completed ED 192 form and any other required documents (if your testing agency no longer has your scores, you need to provide an official transcript or original document showing your scores) to the Bureau of Educator Standards and Certification Waiver Coordinator at the specified address.

After submitting, the Connecticut State Department of Education will review your waiver application. This process takes time, so patience is key. You will be contacted once a decision regarding your waiver request has been made. Ensure all information is accurate and complete to avoid unnecessary delays in processing.

More About Connecticut Ed 192

  1. What is the purpose of the Connecticut Academic Skills Assessment Waiver (ED 192 form)?

The ED 192 form is designed for individuals seeking either formal admission to a Connecticut college or university's educator preparation program or certification in Connecticut who wish to waive the required academic skills tests (Praxis I - PPST). These tests evaluate proficiency in reading, writing, and mathematics. A waiver is granted to those who can provide official documentation showing that they have met the waiver criteria through alternate assessments such as the SAT, ACT, GRE, or PAA scores.

  2. Who needs to complete the Connecticut Academic Skills Assessment Waiver Application?

Individuals who are looking to enter an educator preparation program in Connecticut or are seeking educator certification within the state but prefer not to take the Praxis I tests due to previously demonstrated proficiency on other standardized tests should complete the application. This includes recent graduates, persons changing careers to education, or educators from other states seeking certification in Connecticut.

  3. What are the minimum score requirements for the waiver based on SAT, ACT, GRE, or PAA scores?

The waiver requires minimum scores from one of the following assessments:

    • SAT: A combined score of 1000 (verbal/mathematics), with no less than 400 on each, for tests taken before March 31, 1995. For tests on or after April 1, 1995, a combined score of 1100 is required.
    • ACT: A composite score of 20, with at least 17 in each section for tests taken before October 1989. For tests on or after that date, a composite score of 22 with at least 19 in each section is needed.
    • GRE: A combined score of 1000 (verbal/mathematics) for tests taken before October 1, 2002, and the same score plus a 4.5 in writing for tests taken on or after this date.
    • PAA: Scores equivalent to a 1000 on the SAT, with neither the mathematics nor the verbal subtest score below the equivalent of 400 points.

  4. How can I submit my official score report for the waiver application?

Official score reports must be sent directly from the testing agency to the Connecticut State Department of Education. Test score reports sent by the candidates themselves will not be accepted unless the testing agency no longer retains them. Contact information for each testing agency is provided in the application instructions, and you should request that your scores be forwarded to the Connecticut State Department of Education using the specific program code provided for each test.

  5. What steps should I follow if my GRE scores are older than five years?

If your GRE scores are more than five years old, you must provide an official transcript showing your GRE scores or an original document of the GRE score report that was issued to you, along with your Connecticut Academic Skills Assessment Waiver Application form (ED 192). This is necessary because the Educational Testing Service (ETS) retains GRE scores for only five years.

Common mistakes

When completing the Connecticut ED 192 form for an Academic Skills Assessment Waiver, careful attention is required to avoid common pitfalls that can delay or affect the application process. Observing some frequently made mistakes can help streamline obtaining a waiver, ensuring all prerequisites are effectively met. Here are eight such mistakes to watch out for:

  1. Failing to Use Blue Ink and Uppercase Letters: It is specified that all personal information should be filled out using blue ink and in uppercase letters. Overlooking this instruction may lead to the rejection of the application.

  2. Incorrect Score Reporting: The form requires that official SAT, ACT, GRE, or PAA scores be sent directly from the respective testing company to the Connecticut State Department of Education. Applicants sometimes mistakenly send their score reports, which will not be accepted unless it is a case where the testing agency no longer has the records available.

  3. Not Including Official Score Reports: This form necessitates the inclusion of an official score report with the waiver application. Neglecting to ensure that these scores have been sent or are in the process of being sent by the testing agency amounts to a significant oversight.

  4. Overlooking the Required Supporting Documents: In addition to the score reports, applicants might miss sending the necessary supporting documentation, delaying the process.

  5. Selecting Incorrect Skills Testing Requirement: Sometimes, individuals check the wrong box under the section asking which Essential Skills Testing Requirement is being waived based on SAT/ACT/GRE scores. This misstep can lead to confusion and unnecessary delays.

  6. Providing Incomplete Personal Information: An incomplete application, such as missing personal details like the former last name(s) if applicable, or not providing a contact email address or phone number, can stall the process.

  7. Omission of Date Test Taken: Each testing option requires the date the test was taken. Applicants often leave this field blank or enter an incorrect date, which can invalidate their waiver request.

  8. Incorrect Race/Ethnicity Selection: Though this section is optional, when chosen, it's important to accurately represent one’s race or ethnicity. Incorrect selection does not impact the waiver’s processing but can affect the accuracy of demographic data collected by the Department.

To successfully complete the ED 192 form and avoid these common mistakes, applicants should review their application carefully before submission. Ensuring that all sections are filled out accurately, all necessary documents are included, and official score reports are properly requested from the testing agencies will support a smoother application process.

  • Remember to refer to the specific page on the ED 192 form that details directions for sending an official score report directly from the testing agency to the Connecticut State Department of Education.
  • Double-check personal information for accuracy and completeness.
  • Review the waiver options and ensure the selection correlates with the scores available and the test taken, adhering to the specific date requirements as outlined.
  • Contact testing agencies well in advance to send official score reports to meet application deadlines.

By avoiding these common mistakes and following the instructions meticulously, applicants can enhance their chances of a favorable review without unnecessary delays, moving them one step closer to fulfilling their educational or certification goals in Connecticut.

Documents used along the form

When completing the Connecticut Academic Skills Assessment Waiver Application, commonly known as the ED 192 form, applicants may need to supplement their submission with various other documents and forms to ensure a comprehensive application package. Understanding these supplementary documents is beneficial for a seamless application process.

  • Official SAT/ACT/GRE/PAA Score Reports: These are the test scores from standardized exams that must be sent directly from the testing agencies to the Connecticut State Department of Education. These scores demonstrate the applicant's fulfillment of the academic skills requirement, potentially granting them a waiver.
  • Testing Agency Contact List: This document provides contact information for the SAT, ACT, GRE, and PAA testing agencies. It is used by applicants to request their official test scores be sent to the Connecticut State Department of Education.
  • Prueba de Aptitud Académica (PAA) Score Request Form: Specifically for PAA test takers, this form is used to request that PAA scores are sent directly to the Connecticut Department of Education. It is crucial for applicants who have taken the PAA and are seeking a waiver based on these scores.
  • Abbreviated Conversion Table for PAA and SAT Scores: This table helps in equating PAA scores to their SAT counterparts. It is essential for determining the eligibility of the PAA scores for fulfilling the academic skills assessment waiver requirements.

In summary, each of these documents plays a crucial role in the Connecticut Academic Skills Assessment Waiver Application process. Applicants are encouraged to understand the purpose and requirement of each to ensure they are providing a complete and accurate application packet to the Connecticut State Department of Education for their waiver consideration.

Similar forms

The Connecticut Ed 192 form, titled APPLICATION FOR CONNECTICUT ACADEMIC SKILLS ASSESSMENT WAIVER, is a document used for educators or prospective educators seeking a waiver for academic skills assessment in Connecticut. This document bears similarities to applications used in other jurisdictions for similar purposes, notably in its structure and the kind of information it solicits. For instance, forms like the New York State Teacher Certification Examinations (NYSTCE) Safety Nets and the California Basic Educational Skills Test (CBEST) Waiver Application also provide pathways for educators to bypass certain testing requirements based upon previously demonstrated competencies through other standardized tests.

Both the NYSTCE Safety Nets and the Connecticut Ed 192 form serve a similar audience - educators wishing to demonstrate competence without the need for additional testing. The NYSTCE Safety Nets allow candidates who have not passed a Content Specialty Test (CST) or the Educating All Students (EAS) test to demonstrate competency through alternative examinations or coursework. Just like the Ed 192 form, the NYSTCE Safety Nets application requires detailed personal information, documentation of educational background, and scores from alternative assessments. However, the NYSTCE offers more varied pathways and alternatives, reflecting the differences in state educational standards and requirements.

On the other hand, the California CBEST Waiver Application shares the goal of permitting educators to bypass basic skills testing under specific conditions, echoing the Ed 192 form's intent. The CBEST Waiver is applicable to individuals who have demonstrated proficiency through other standard examinations such as the SAT, ACT, or GRE, similar to the prerequisites found in the Ed 192 document. Both applications necessitate the submission of official score reports directly from the testing agency, ensuring the integrity and verification of scores. Nonetheless, the CBEST Waiver focuses strictly on basic skills (reading, writing, and mathematics), whereas the Ed 192 form encompasses a broader range of competencies, including ESLAT and TOEFL scores for candidates with English as a second language.

Dos and Don'ts

When preparing the Connecticut ED 192 form, consider adopting a cautious and thorough approach to ensure accuracy and compliance. Below are essential dos and don'ts to guide you through the process:

  • Do ensure that all personal information is filled out using blue ink and uppercase letters, as this helps maintain the clarity and legibility of your submission.
  • Do double-check that your Social Security Number, birth date, and contact information are correctly entered to avoid any processing delays due to inaccuracies.
  • Do provide an official score report from the testing agency directly; remember, copies sent by candidates themselves are not acceptable.
  • Do verify that your official scores meet the minimum requirements set by the Connecticut State Department of Education before submitting your application.
  • Do review every section of the application for completeness, especially the section regarding the Essential Skills Testing Requirement, to ensure you check the correct box corresponding to your qualifying exam.
  • Don't leave any sections blank. If a section does not apply to you, it's prudent to fill it with "N/A" to indicate that you have reviewed the section and determined it to be not applicable.
  • Don't forget to sign and date your application. An unsigned application is incomplete and will not be processed.
  • Don't ignore the testing agency contact list provided. Use it to ensure that your score reports are sent correctly and promptly to the Connecticut State Department of Education.
  • Don't hesitate to contact the Bureau of Educator Standards and Certification if you have any questions or concerns about the application process. It's better to seek clarification than to submit an incorrect or incomplete application.

Approaching the ED 192 form with diligence and attention to detail can smooth the path toward your academic and professional objectives. Ensure to adhere to the guidelines provided for a successful application process.

Misconceptions

Understanding the Connecticut Ed 192 Form can be a bit confusing, leading to some common misconceptions. Let's address some of these misconceptions to ensure that applicants have the correct information.

  • Only Teaching Candidates Need This Form: While it's true that the form is often used by prospective educators, it's important to remember that the Connecticut Academic Skills Assessment Waiver Application is relevant for a broader range of candidates in the education field, not just teachers. This includes those looking to fulfill academic skills requirements for various certification processes within Connecticut's Department of Education.
  • Any Test Scores Can Be Submitted by the Candidate: There's a misconception that candidates can submit any test scores directly. However, the form specifies that only official score reports sent directly from the testing agencies (SAT, ACT, GRE, PAA) will be accepted. Scores forwarded by candidates themselves will not be processed, unless the testing agency no longer has those scores on file.
  • You Must Complete the Entire Form Yourself: While the majority of the form should indeed be filled out by the applicant, there are parts, such as the official score report request, that involve direct coordination from the testing agencies. This means you'll initiate the request, but the agency sends the scores directly to the Connecticut State Department of Education.
  • Any Past Scores Are Acceptable: Some believe that any previous test scores, regardless of when they were taken, can be used to apply for the waiver. The truth is that there are specific date ranges and score thresholds for SAT, ACT, GRE, and PAA scores to be considered valid for the application process. Scores outside these parameters may not fulfill the academic skills waiver requirements.
  • Submitting This Application Guarantees a Waiver: Merely submitting the Connecticut Academic Skills Assessment Waiver Application does not guarantee that a waiver will be granted. The waiver is awarded based on meeting specific criteria outlined in the form, including but not limited to achieving minimum scores on accepted standardized tests.
  • The Form Is Only Available in English: While the primary instructions and form are provided in English, it's crucial to note that there are components of the process, such as the PAA Score Request, which cater to Spanish-speaking applicants. This reflects an effort to accommodate a diverse range of candidates applying for the waiver.

In summary, when applying for the Connecticut Academic Skills Assessment Waiver, it's vital to clearly understand the form's requirements and processes. Misinterpretations can lead to delays or rejection of the waiver application. Applicants should thoroughly review the provided instructions and seek clarification when necessary to ensure a smooth application process.

Key takeaways

Whether you're a prospective educator looking to apply for a Connecticut Academic Skills Assessment Waiver or you're simply seeking guidance on how to properly complete the application process, understanding the intricacies of the ED 192 form is crucial. Here are five key takeaways that will ensure your application journey is smooth and successful:

  • Complete the Application Accurately: The importance of filling out the ED 192 form with precision cannot be overstated. Ensure that all your personal information is printed in blue ink and uppercase letters. This not only includes your name and contact information but extends to your academic history and your choice for the essential skills testing requirement waiver based on SAT, ACT, GRE, or PAA scores.
  • Official Score Reports are Mandatory: One of the fundamental requirements is the submission of official score reports alongside your application. It's critical to note that scores sent by waiver candidates directly are not accepted. Instead, scores must be sent directly from the respective testing agencies. This ensures the authenticity of your application and accelerates the processing time.
  • Understand the Minimum Standards: Familiarizing yourself with the minimum standards for an Academic Skills Assessment Waiver is essential. Whether your scores come from SAT, ACT, GRE, or PAA exams, knowing the cut-off scores helps in understanding whether you qualify for the waiver. Keep in mind that if your scores are from multiple attempts, the highest verbal and mathematics scores will be combined for evaluation.
  • Authenticating Your Scores: It’s crucial to contact the appropriate agencies to send your scores to the Connecticut State Department of Education. Each testing agency (College Board for SAT, ACT, Inc. for ACT scores, ETS for GRE, and the College Board in Puerto Rico for PAA) has specific instructions and forms available online or through customer service. Remember, the application won’t be processed until these scores are received officially.
  • Navigate the Waiver Application Effectively: The waiver application process is structured to identify candidates who meet the academic skills requirement through alternative means. Paying close attention to the application directions provided, especially on Page 6, and ensuring that all parts of the form are completely and correctly filled out, will facilitate a smoother review process. Avoid delays by double-checking for any incomplete sections or missing documents.

Embarking on the journey to become an educator in Connecticut is an exciting path. By meticulously preparing your Connecticut Academic Skills Assessment Waiver Application, you’re not just complying with procedural necessities but also showcasing your commitment to professionalism and excellence in education.
